Purpose
The Pediatric RN works in partnership and joint accountability with other staff members to achieve Neighborcare’s Mission, Guiding Principles and goals. The primary focus of the RN is providing age-appropriate care for children with a variety of physical and mental disabilities and/or acute and chronic medical and surgical conditions. The Pediatric RN also collaborates with the patient and family to best promote healthful practices and disease prevention.While RNs will be assigned to a specific clinic site, they will also work in partnership with each other to meet the needs of patients throughout Neighborcare. The ANA Standards of Clinical Nursing Practice and the AAACN Ambulatory Care Nursing Administration and Practice Standards provide a basis for the practice of the registered nurse in the ambulatory setting. Uses the nursing process to assess needs, plan care, implement the plan, and evaluate the patient's responses and progress towards goals.

In this position you will:
Health Promotion & Case Management - 40%
- Maintain a registry of medically-complex children in the practice to proactively insure that they receive all required or anticipated care
- Facilitate care coordination between outside care providers and internal clinic team to insure care providers are aware of the child's current condition, needs and medical management priorities (through EHR or case conferences)
- Develop partnerships with patients and families and facilitate continuity of care between patients and primary care providers (residents) for positive outcomes in health status
- Identify patient and family goals for improving health status
- Assess patient and family’s understanding of health and readiness to change
- Provide patient and family with education about development and disease processes and offer tools and skills needed to achieve self-management goals
Patient and Family Education - 30%
- Educate patients and families in the use of medications
- Educate patients and families regarding abnormal laboratory results requiring education and follow-up
- Provide patients and families with information to assist with self-care
- Promote wellness and the importance of preventive care
- Provide motivational interviewing and harm reduction counseling
Triage Emergent Health Issues and Perform Select Clinical Procedures - 20%
- Perform telephone or in-person triage and home care advice for pediatric patients in accordance with Telephone Triage Protocol references
- Insert and remove urinary catheters
- Administer select injections and medications
- Perform and assist with procedures as appropriate by licensure and Neighborcare protocol and policy
Working in Partnership and Joint Accountability - 10%
- Pediatric RN will be assigned to a specific clinic to manage defined panels of patients and work as members of Neighborcare’s Primary Care Nursing Team
- Maintain joint accountability with the clinic team and the medical program as a whole, to include working at a variety of sites to best meet the needs of the patients
- Work with others to assess and redefine priorities as needed, and support colleagues throughout the organization either on site or remotely.
- Work in partnership with care team members to plan and communicate care delivery
- Delegate tasks to Medical Assistants as appropriate
- May provide supervision of or training for students who are assigned to Neighborcare as part of their professional education
Skills, Knowledge & Abilities:
- Knowledge and understanding of developmental milestones, common illness/diseases of childhood, and mental health problems with the ability to work effectively with patients
- Knowledge of community health care resources
- Knowledge of medical terminology and clinical procedures; anatomy, physiology, biology, human growth and development; asepsis and universal precautions
- Knowledge, skill and ability to perform CPR
- Knowledge of and skill in the use of personal computers and the ability to learn and use company software programs and reports
