About the role

Description

The Children's Home is seeking a Full-Time Hospital Services Coordinator.  The Hospital Services Coordinator is a member of the Care Coordination Team and will assist in the coordination and organization of all day-to-day functions in the hospital, daily maintenance of all medical records, and responsible for efficient and accurate scheduling of in-patient and outpatient services through the various programs of The Children’s Home. This position will also work closely with the Manager of Care Coordinator/RN Admissions Case Manager in the planning and admitting of new patients to the Pediatric Specialty Hospital.

Essential Functions:

1. Coordinates the maintenance of the medical record for all patients within The Children’s Home.

2. Ensures that all new patient information is obtained and processed with appropriate admitting paperwork for admission of patient, including assigning the patient’s medical record number and processing initial insurance verification.

3. Ensures that all test results/reports are in the patient’s chart once reviewed.

     a. Gathering test results from the CRNP office, scanning into EMR, and obtaining results that have not been received.

     b. Assists with reconciling lab bills.

4. Compiles infection control data for submission to Quality and Compliance Manager.

5. Under the direction of Manager of Care Coordination/RN Admissions Case Manager and Clinical Leaders, sets up ambulance transports for scheduled and unscheduled admissions, discharges, and tranfers providing EMS with authorization as available.

6. Ensure that all patient charts are assembled in an appropriate manner.

7. Coordinates the review process of all medical records at time of discharge, ensuring that all necessary information is completed and in compliance with PA State regulations.

8. Interacts with the RN Case Manager/Utilization Review to ensure that all appointments at outpatient facilities are scheduled and required documentation obtained.

9. Prepares Med Exec packet and transcribes meeting minutes each month.

10. Coordinates administrative and clerical functions as needed under the direction of the Chief Nursing Officer, Manager of Nursing Services and the Manager of Care Coordinator/RN Admissions Case Manager.

11. Schedules for appointments for various services, including therapies, assessments, etc.

12. Ensures all program-specific patient demographics and insurance information are obtained prior to appointment and accurately entered into the electronic medical record system in a timely manner.

13. Maintain patient records ensuring all required information is obtained and entered and/or scanned into the patient’s chart.

14. Follow-up on any outstanding insurance issues for inpatient care and other services to ensure accurate invoicing and communicate closely with RN Case Manager/Utilization Review and Accounts Receivable Associate.

15. Responsible for creating, maintaining, and retrieving patient records, obtaining information required for authorizations, retrieving information needed for reviews and appeals, and other department projects.

16. Provides support for front desk reception duties.

Requirements

Minimum Skills and Abilities Required:

1. High school diploma or equivalent required.

2. Completion of medical secretary/admissions/registration program, or other related program, or 2-4 years of experience in a medical field.

3. General knowledge of computer software and electronic medical records.

4. General knowledge of Microsoft Excel.

5. Works independently and efficiently, possessing a high attention to detail and maintaining the volume of scheduling clients.

6. Strong organization, time management, and interpersonal skills.

7. Interpersonal and communication skills necessary to effectively communicate with patients, staff and other personnel.
