The Opportunity
The Impact Team is looking to hire a talented health and wellness coach skilled in behavior change techniques and basic knowledge in nutrition, movement, sleep, stress management, and overall well-being to provide virtual coaching to our patients within the Impact program. The Impact program is One Medical’s chronic care management program designed to support and guide members who are in need of behavior change support to improve overall wellbeing and manage chronic disease (diabetes, obesity, hypertension, ect.). Members who receive coaching have been identified by the PCPs and work collaboratively within the Impact care team (PCP’s, RN’s, Pharmacists).
Position Summary:
This role works directly with patients, helping support and guide them through the process of behavior change. The foundations of health coaching are motivational interviewing, active listening, and facilitating learning, change, and positive progress toward health goals and improved outcomes.
Health coaches quickly build rapport and structure a conversation with patients that engages the patient in self-inquiry by asking powerful questions that lead them to identify their source of motivation, insight into their self-efficacy, obstacles that get in the way of achieving goals, etc. Coaching believes the patient is the expert on the patient and inspires and empowers the patient to explore this reality to make effective change.
Tactically, you help patients clarify their personal goals, create a personalized path forward, and encourage them to be an active participant in their health.
Progressively, you help patients explore resistance, build motivation, and inspire breakthroughs.
You also model the behavior of calm confidence and energized humility. Through your demeanor and own life experience, you demonstrate hope and the capacity to live a healthful life.
Coaches will work with a variety of One Medical members but can expect to work primarily with folks ranging in age from 20-70 who are interested in making changes to their health by focusing on the primary pillars of health: stress, sleep, exercise, and diet. The health of members engaging in coaching will range from health optimization to prevention/progression of disease to active management of disease.
Responsibilities:
- The coach co-creates individualized action plans and SMART goals with each patient to support them in achieving their personal health goals
- They are responsible for facilitating the process of coaching for both individuals and groups, via zoom
- Provide tailored one-on-one 30-minute health coaching sessions
- Understand the patient's health conditions related to their coaching referral, leveraging expertise from the Impact care team (PCP’s, RN’s, Pharmacists) as needed
- Provide patients with resources via our online platform Pillar
- Maintain accurate and up-to-date documentation of all patient encounters in 1Life and Innovaccer
- Manage a patient caseload, scheduling initial and follow-up sessions, outreaching when patient has canceled/no-showed, responding to messages and tasks in a timely manner
- Facilitate group health coaching in coordination with a PCP facilitator within our Living Better Group visit
- Continually work with and collaborate with the Impact team and PCP to share successes, provide feedback, and round on difficulties
- Prepares for, attends and participates in team meetings, huddle(s), and rounds
- Supports special projects such as the One Medical School of Coaching, and collaborations with Amazon
Position Requirements:
- Minimum of 3 years of experience working as a Health Coach
- National Board-Certified Health and Wellness Coach (NBC-HWC)
