Position Overview
The ILP Program Manager will provide monitoring to ensure services are provided to meet contractual requirements. They will directly supervise staff that are employed to work within Independent Living housing and will provide contract-based services. The ILP Program Manager will oversee staff that will assist participants in identifying and advocating for their needs and connect participants to community resources. In addition, the ILP Program Manager will support permanency planning and will attend Family Team Meetings, court hearings, treatment and recovery appointments, and other events with participants.
Responsibilities:
- Provide supervision with staff based on contract requirements to be able to assess and individualize work responsibilities, goals, and job tasks.
- Organize and hold team/support meetings and individual supervision meetings with all staff for in-service training, case consultations, support, and feedback.
- Provide training/education to MDHHS local offices about contract services provided.
- Work in conjunction with the Director in developing and maintaining the integrity of programs in accordance with licensing rules and regulatory bodies.
- Work in conjunction with Director in identifying, recruiting, and training community representatives to become collaborative members for contract-based services.
- Serves as the point person for referrals and intakes. Will maintain a referral log.
- Responsible for the development and oversight of day-to-day activities and program processes. Completes maintenance and IT requests as necessary.
- Will ensure census is maintained provided by management.
- Assist with the development and continuous monitoring of tracking systems that measure performance outcomes.
- Flexibility to attend meetings and co-facilitate groups including non-standard working hours such as evenings and weekends in accordance with client’s needs.
- Responsible for scheduling and monitoring of HRIS activities and performance reviews of all staff.
- Ensures all staff are following agency policies, site procedures, and treatment plans of participants.
- Implements Mi-Team practices in daily activities including teaming, engagement, assessment, and mentoring.
- Provide 24-hour crisis on call availability to treatment team on an as needed basis.
- May be required to work shifts on an as needed basis.
- May be required to transport participants on an as needed basis.
- Performs other duties related to the essential duties of the position and within the individual’s capabilities as assigned by one’s supervisor.
