About the role

DUTIES AND RESPONSIBILITIES:

Clinical Leadership

  • Provide overall leadership and direction for all nursing services within the skilled nursing facility.
  • Maintain accountability for the quality, safety, and effectiveness of resident care.
  • Provide clinical oversight and consultation to the Adult Day Health Care Center nursing staff and programming.
  • Ensure resident-centered care is delivered according to evidence-based practices.
  • Promote a culture of dignity, compassion, and person-centered care.

 

Regulatory Compliance

  • Ensure compliance with all applicable CMS, New York State Department of Health, OSHA, CDC, and organizational regulations and standards.
  • Lead preparation for annual surveys, complaint investigations, and regulatory inspections.
  • Develop and implement Plans of Correction as necessary.
  • Monitor compliance with infection prevention, medication management, documentation, resident rights, and quality measures.

 

Quality Assurance & Performance Improvement (QAPI)

  • Lead the Nursing Quality Assurance Performance Improvement (QAPI) program.
  • Monitor clinical outcomes including:
    • Falls
    • Pressure injuries
    • Weight management
    • Infections
    • Hospital readmissions
    • Medication errors
    • Behavioral health outcomes
  • Analyze quality metrics and implement corrective actions.
  • Conduct regular audits of documentation and clinical practices. 

Staffing & Workforce Management

  • Develop staffing plans that meet regulatory requirements and resident acuity.
  • Maintain required staffing levels, including achievement of target nursing hours per resident day.
  • Recruit, hire, orient, coach, evaluate, and retain nursing personnel.
  • Conduct annual performance evaluations and competency assessments.
  • Promote employee engagement, accountability, and professional development.
  • Monitor overtime, agency utilization, absenteeism, and labor productivity.

Clinical Operations

  • Oversee:
    • Nursing assessments
    • Care planning
    • Medication administration
    • Treatment services
    • Wound management
    • Infection prevention
    • Behavioral health interventions
    • Restorative nursing
  • Ensure timely and accurate clinical documentation.
  • Collaborate with rehabilitation, social work, dietary, pharmacy, and medical providers to coordinate care.

 

Adult Day Health Care Clinical Oversight

  • Provide clinical leadership and oversight of nursing services within the Adult Day Health Care Center.
  • Ensure compliance with all Adult Day Health Care regulations.
  • Review participant clinical status and care plans.
  • Collaborate with the ADHC leadership team regarding participant safety, clinical programming, and quality outcomes.
  • Support survey readiness and quality improvement initiatives for the program.

 

Resident & Family Experience

  • Foster positive relationships with residents, participants, families, physicians, and community partners.
  • Address concerns promptly and professionally.
  • Promote resident and participant satisfaction through responsive leadership and continuous improvement.

Financial Stewardship

  • Manage the nursing department budget.
  • Monitor labor expenses, supply utilization, and operational efficiencies.
  • Participate in annual budget development.
  • Identify opportunities to improve quality while managing costs responsibly.

 

Emergency Preparedness

  • Participate in emergency preparedness planning.
  • Lead nursing response during emergencies.
  • Ensure staff competency in emergency procedures and disaster preparedness.

 

Leadership & Collaboration

  • Participate as a member of the Executive Leadership Team.
  • Support organizational strategic initiatives.
  • Foster interdisciplinary collaboration.
  • Serve as Administrator-on-Call as assigned.
  • Lead departmental meetings and clinical committees.
  • Other duties as assigned.

  • Ability to interact with seniors, including those with dementia, and their families in a supportive way.
