Cooperative for Human Services Inc. is a multi-service organization that provides residential services, independent living supports, and guardianship services for adults with intellectual, developmental and other related disabilities. We are looking for a compassionate and patient Individual Support Services Coordinator to join our team! This position involves working directly with adults with disabilities who live independently.

Job Description
- Provide direct supports to assigned individuals within their home
- Assist with grocery shopping, medical appointments, and other appointments within the community
- Assist in the development of each person’s ISP goals and objectives that reflect the preferences, needs, and desires of the persons served in conjunction with members of his or her support team.
- Maintain case records in accordance with CHS and the regulations of the Department of Developmental Services.
- Act as a liaison to and communicate on a regular basis with the individual served, family members and/or guardians, other service providers
Schedule: Multiple full-time shifts available. Weekends and weekdays. Typical shifts are 10 AM-6 PM, 11 AM-7 PM, or 12 PM-8PM with rotating overnight on-call responsibilities.

Requirements
- First Aid and CPR Certification required before starting employment - we will help you obtain this!
- High school diploma or equivalent
- Valid U.S. driver's license and access to a vehicle
- Ability to read and write in English
- Ability to lift up to 30 lbs
- Familiarity with computers
- Excellent verbal and written communication skills
- Demonstrated ability to work independently
- Ability to utilize excellent judgement in developing sound solutions
- Ability to exhibit patience with others
