Requirements:- Graduate of an accredited school of nursing.- One to two years of recent acute care experience in an institutional setting.- Current licensure in the State of Arizona.- BLS hands-on CPR certification.- Valid driver’s license and auto insurance.- Associate degree with one year of home health care experience preferred.
Responsibilities:- Initiate and revise the plan of care based on patient status and needs.- Develop a comprehensive care plan with patient and family involvement, focusing on palliative nursing actions.- Provide counseling to patients and families to meet nursing and related needs.- Deliver health care instructions tailored to each patient's assessment and plan.
