About the role

The Clinical Manager is primarily responsible for the day-to-day supervision of home health clinical services. The Clinical Manager establishes, implements, and evaluates goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organization and philosophy.

 

Position: Clinical Manager – for Home Health

Salary: $92,000 - $100,000/annually, depending on experience

Service Area: Twin Cities Metro area. Plus, mileage reimbursement of 72.5 cents per mile.

Schedule: 1.0 FTE, Full - Time / Monday – Friday, 8:00am-5:00pm (weekend rotation: every 8-9th weekends, holiday rotation: 1-2 per year and on-call rotation: shared responsibilities with the other CLMs)

Position Functions & Responsibilities:

  • Coordinates and oversees all direct and indirect client services provided by clinical organization personnel.
  • Provides guidance to the clinical team to assist them in continually improving all aspects of home health care services provided through organization personnel.
  • Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and maintains compliance to the Lifespark Home Care standards.
  • Manages clinical team.
  • Provides help in assessment, planning, implementation and evaluation of client and family/caregiver care to all clinical personnel as indicated.
  • Completion of assigned HCHB workflow tasks including episode management.
  • Oversees the maintenance of client clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
  • Fostering strong relationships with clients and referral sources. Addresses and resolves complaints efficiently to ensure a satisfactory outcome.
  • Assures proper maintenance of clinical records in compliance with local, state and federal laws
  • Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of client services.
  • Interprets operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues or expenses.
  • Evaluates performance of clinical team.
  • Develop skills and techniques in evaluating the performance of clinicians.
  • Hire, evaluate, coach, and terminate organization personnel.
  • Conducts clinical performance evaluations annually, or more frequently if indicated.
  • Plans and implements in-service and continuing education programs to meet education and training needs of organization personnel.
  • Assists with the organization QAPI programs including evaluation of organization performance via performance improvement programs, productivity, quarterly and annual reviews.
  • Assures the quality and safe delivery of home health services provided through the organization.
  • Develops, implements and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel, either directly or by delegating to another staff member.
  • Assists in the development of organization goals. Develops, recommends, and administers organization policies and procedures.
  • Stays informed about changes in the field of nursing, therapies and home health care; shares information with appropriate organization personnel.
  • Promotes home health referrals in the health care community.
