Job Title: Advanced Practice Provider

What You’ll Champion:

• Seeking a Primary Care Family Nurse Practitioner (FNP) to support a Community Health Center dedicated to improving access to care in York County.
• Provide continuity of care for an existing and growing patient population.
• Manage and support an established patient panel, including individuals receiving Medication‑Assisted Treatment (MAT).
• Help establish care for a vulnerable and underserved patient population in a mission‑driven outpatient setting.
• Deliver both in‑person and virtual primary care services, supporting flexible access for patients.
• Collaborate within a supportive care team consisting of two CRNPs and one physician.
• Practice in a team‑based, integrated primary care environment focused on patient‑centered outcomes.
• Schedule:
  • Monday: 8:00 AM – 4:30 PM
  • Tuesday: 11:00 AM – 7:00 PM Includes virtual patient care hours from 5:00 PM – 7:00 PM
  • Wednesday: 8:00 AM – 5:00 PM
  • Thursday: 10:00 AM – 2:00 PM
  • Friday: 8:00 AM – 4:30 PM
