About the Role:
We are seeking a Clinical Program Manager (Nurse Practitioner) to serve as the cornerstone of Motion MSK’s Fracture Prevention Program. This program is dedicated to helping patients at risk for fractures by providing timely screenings and follow-up care. This program plays a vital role in ensuring patients receive the attention they need to improve their bone health and reduce future health risks.
This is a unique hybrid role that blends Nurse Practitioner-level case management with clinical program ownership, ideal for someone who is energized by both direct patient care and building scalable care models.
You will operate at the intersection of patients, partner PCPs, and program strategy - delivering high-quality clinical care while designing and evolving the systems that support it.
This role requires strong clinical judgment, operational thinking, and the ability to orchestrate multiple moving parts in a dynamic healthcare environment. In many ways, you are both the architect of the program and a core clinical provider within it.
What You’ll Do:
Program Oversight & Development:
- Design, implement, and continuously refine program workflows, care pathways, and operating models to improve patient outcomes and program performance
- Partner closely with Clinical Product to shape patient journeys, protocols, and ongoing program evolution
- Serve as the central point of coordination across stakeholders, including patients, partner PCPs, internal clinical and operational teams, and leadership
- Own program execution, ensuring alignment to defined goals, service commitments, and regulatory requirements
- Coordinate program logistics, resourcing, and scalability as the program grows
- Play a key role in new client onboarding and program configuration, acting as a subject matter expert internally and externally for the Fracture Prevention Program
Patient Care & Clinical Management
- Provide direct patient care focused on fracture risk identification, prevention, and longitudinal bone health management
- Educate patients on osteopenia and osteoporosis, including treatment options, risks, and follow-up care
- Conduct risk factor assessments and coordinate appropriate screenings (e.g., DXA scans), including communicating results and next steps
- Initiate and manage treatment plans in alignment with clinical protocols, including oral bisphosphonates, IV zoledronic acid, and Prolia
- Collaborate with partner PCPs to enter orders and ensure coordinated, compliant care delivery
- Guide shared clinical decision-making across Motion MSK and external providers
- Identify appropriate patients for program inclusion and ensure accurate data collection and documentation
Team Leadership & People Management:
- Lead and support a Patient Coordinator, with ownership over hiring, onboarding, and training as the team scales
- Build a high-performing, patient-centered team culture grounded in accountability, collaboration, and service excellence
- Establish and maintain SOPs, SLAs, and performance metrics to ensure consistency and quality
- Provide ongoing coaching, feedback, and professional development
- Deliver training as workflows evolve and new capabilities are introduced
- Oversee daily operations, including team coverage, escalations, and workflow management
Location & Schedule:
This position is fully remote, ideally located in Florida, with the ability to work East Coast hours. Patient care hours are 8am - 5pm ET Monday - Friday.
Requirements
Who You Are:
- Active, unrestricted Nurse Practitioner (APRN) license in the state of Florida, or ability to obtain prior to start
- National board certification as a Nurse Practitioner (e.g., AANP, ANCC or equivalent)
- Eligible for prescriptive authority in Florida
- Must be registered as an Autonomous APRN in Florida, or meet all eligibility requirements and be able to obtain autonomous registration upon hire.
- 3+ years of clinical experience, with a minimum of 2 years of clinical case management experience
- Strong clinical judgment with the ability to assess risk, initiate treatment plans, and guide longitudinal care
- Experience coordinating care across providers, including collaboration with primary care physicians and external partners
- Comfortable working in a structured, protocol-driven environment while applying sound clinical decision-making
- Collaborative and adaptable, with an interest in shaping and improving clinical and operational processes
