Patient Navigator Oncology Full Time Days- BHS

Summary

Under general administrative direction, manages the non-clinical care of patients across the oncology service line in conjunction with the patient’s multidisciplinary care team. Collaborates with the patient navigator in coordinating referrals from physicians and health care facilities for patients; providing a significant level of patient education related to their illness and planned treatment. Directs patients through the complex care continuum and assuring patients receive care that consistently follows evidence-based standards and is in accordance with regulatory standards as defined by the regulatory agencies. Identifies and interacts with community resources and develops relationships with service providers to enhance the patient experience and eliminate barriers to successful completion of timely care. Manages national accreditation program standard compliance by ensuring staff accountability. Facilitates administrative activities in support of the service line staff and goals. Completes established competencies for the position within designated introductory period. Other related duties as assigned.

EDUCATION:

Required:  Bachelor’s Degree from an accredited college in business or healthcare related field
Preferred:  Master’s Degree from an accredited college or university in business or healthcare related field

EXPERIENCE:

Required:  3 years of experience in a related field
Preferred:  5 years of experience in a related field

CERTIFICATIONS:

Preferred:  Certified Navigator-Breast Advocate (CN-BA), Certified Health Education Specialist (CHES)
