Responsibilities:

  • Makes initial contact and physical, social and psychological and financial assessment of patients to determine appropriateness of hospice and home health services.
  • Completes initial nursing assessment and plan of care and completes routine visits when needed.
  • Works with referral partners to provide appropriate alternatives for patients who do not meet criteria including patients with no funding source.
  • Assists with patient’s insurance to determine coverage.
  • Ensures accurate authorizations for disciplines.
  • Interprets policies and acts as resource for case managers, physicians, hospital and community for effective public relations and marketing of services.
  • Acts as liaison between physicians, skilled nursing facilities, hospitals and rehabilitation providers communicating service enhancements, new products, new programs and service delivery.
  • Facilitates communication between Business Development and Hospital, Skilled Nursing Facility representatives, and other referral sources. Relates opportunities for recovery activities.
  • Understands that teamwork is a crucial part of our business, and is able to work well with colleagues, no matter the circumstances. Using individual skills, cooperates with others, and accepts and provides constructive feedback despite any personal conflicts between individuals involved.

Requirements


1. Graduate from an accredited school of nursing as an RN. BSN preferred; and current license in the state of Colorado.


2. Two years of experience working in an acute care setting; one year of hospice experience preferred. Working knowledge of community resources. Ability to meet, collaborate and work efficiently with others in the community. Knowledge of entitlement programs.


3. Must show Proof of a Tuberculin Skin Test (PPD), TB blood test, and/or CXR, or get one upon employment (An annual assessment for symptoms must be completed thereafter)

  • Proof of immunizations for MMR (X 2 if born after 1957), D/T and Varicella is recommended, but not required
  • It is a requirement for all Sangre employees to be vaccinated against seasonal influenza.

4. Must possess a valid driver’s license issued by the state of Colorado.

  • Maintain insurance in accordance with state and/or organization requirements and maintain automobile in good working order
  • Must meet minimum driver standards; driver’s license must have no physical restrictions other than corrective lenses; no more than two moving violations in the last 36 months and no major charges, such as reckless driving, driving while intoxicated/driving under the influence o
