What You’ll Do

• Deliver skilled nursing care in alignment with individualized resident care plans

• Administer medications and treatments accurately and safely

• Monitor and document resident conditions, reporting promptly to RNs and physicians

• Collaborate with interdisciplinary teams to ensure quality outcomes and resident well-being

• Provide guidance and leadership to CNAs to maintain high standards of care

• Help foster a safe, clean, and supportive environment for residents and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license in Indiana (required)

• CPR certification required

• Experience in skilled nursing, rehab, or long-term care preferred

• Strong clinical judgment and attention to detail

• Compassionate, reliable, and team-oriented personality

• Excellent communication and interpersonal skills
