This role <span>manages inter‑facility transfers</span> by integrating advanced clinical assessment, situational awareness, <span>and real‑time decision-making</span> to ensure patients are placed at the appropriate level of care in a timely and safe manner. Acting as the first clinical point of contact <span>for referring hospitals,</span> providers, and healthcare facilities, the RN evaluates patient acuity, <span>determines appropriate</span> <span>clinical pathways,</span> <span>and anticipates potential</span> risks during transfer or transport. </h3><h3></h3><h3>The Patient Logistics RN functions as a critical communication hub—linking referring providers, accepting clinicians, receiving units, transport teams, and family members—to <span>ensure accurate,</span> comprehensive, and clinically relevant information flows between all parties. This role requires strong clinical judgment, rapid prioritization skills, and the ability to navigate bed capacity constraints, operational limitations, and emergent care needs. </h3><h3></h3><h3>Through diligent attention to regulatory requirements, specifically EMTALA and other transfer-related standards, the Patient Logistics RN upholds patient safety, compliance, and continuity of care. <span>By optimizing resources, advocating</span> for patients, <span>and facilitating efficient</span> transitions, this role significantly influences patient outcomes, throughput performance, and overall healthcare system efficiency. </h3><p></p><p><b>ESSENTIAL FUNCTIONS</b></p><h3>1. Facilitate and coordinate patient transfers between healthcare facilities, departments, or units</h3><h3>2. Work in a fast-paced, ever changing working environment.</h3><h3>3. Conduct clinical assessments of transfer requests to determine appropriate level of care needed</h3><h3>4. Communicate effectively with physicians, nurses, and staff at both sending and receiving facilities</h3><h3>5. Manages multi-phone call activity to minimize any delays experienced by customers</h3><h3>6. Document all transfer information accurately in electronic health records and transfer systems</h3><h3>7. Coordinate transportation arrangements including ground and air ambulances when necessary</h3><h3>8. Monitor bed availability and collaborate with bed management teams</h3><h3>9. Serve as a clinical resource for transfer-related questions and concerns</h3><h3>10. Participate in quality improvement initiatives related to transfer processes 11. Demonstrates knowledge of the use of emergency notification system (PULSARA) and takes initiative to activate system for emergency transfer of patients with STEMI, stroke, trauma, ECMO.</h3><h3>12. Takes the initiative to anticipate problems, uses creative thinking and generates reports as requested.</h3><p></p><p><b>MINIMUM QUALIFICATIONS</b></p><h3>1. Must possess a comprehensive understanding of the Paramedic scope of practice, including all clinical procedures, patient care standards, and legal/regulatory requirements governing emergency medical services. This knowledge should encompass state and local protocols, documentation standards, infection control practices, and the ability to apply these principles effectively in both pre-hospital and hospital settings. Additionally, the Paramedic should demonstrate proficiency in recognizing limitations of the role and collaborating with other healthcare professionals to ensure safe, high-quality patient care.</h3><h3>2. High school diploma or equivalent (GED).</h3><h3>3. Graduate of an approved Paramedic course with a current Iowa Paramedic (PM) certification, or an Iowa RN Exemption or Illinois PHRN certification, based on role. (MercyOne Genesis – current Paramedic (PM) license in either Iowa or Illinois is required at the time of hire. A secondary PM license for the other state must be obtained within 120 days of the hire or transfer date.)</h3><h3>4. Minimum of 2-3 years of clinical experience.</h3><h3>5.
