What are we looking for?
We want to add a medical appointment specialist direct support professional (DSP) to our team. The medical appointments specialist DSP coordinates and supports the healthcare needs of individuals with intellectual and developmental disabilities (IDD). This role involves transporting individuals to medical appointments, communicating with healthcare professionals, maintaining accurate and timely documentation, and ensuring follow-up care is completed. The ideal candidate will have strong organizational skills, the ability to problem-solve, and a commitment to enhancing the health and well-being of the people we support. You will collaborate closely with the lead DSP, program director, and state director to ensure that SJCS’s brand voice and messaging are consistently represented across all materials.
Your overall essential responsibilities will be the following:
Medical Appointment Coordination
- Transport individuals to and from medical, dental, and therapy appointments in a safe and timely manner.
- Serve as the primary point of contact during appointments, advocating for the individual and ensuring their needs are addressed.
- Communicate effectively with healthcare providers to gather and understand diagnosis, treatment plans, and instructions.
Documentation & Communication
- Document all appointment details, including diagnoses, treatment plans, and follow-up recommendations, in Therap or other designated systems within required timeframes.
- Ensure accurate and comprehensive health-related updates are shared with the individual’s support team, including Residential Team Leaders, Program Managers, and family members as needed.
