Job Duties:

You’ll be at the center of patient flow, partnering with referring and accepting physicians to coordinate smooth, efficient transfers between facilities. You’ll also work closely with attending physicians and inpatient charge nurses to ensure patients are placed quickly and appropriately, aligning with care needs and placement guidelines.

  • A typical schedule is full-time, 40 hours weekly, nights.

  • Shift differential applies to applicable hours.

  • At least three (3) years of RN work experience is required.

  • Opportunity to work hybrid following a 6-month in office orientation. Additionally, in-office work is required as needed.

Position Details:

Works in collaboration with referring and accepting physicians to facilitate effective and efficient transfers of patients between health care facilities. Works with attending physicians and inpatient unit charge nurses to facilitate timely and appropriate bed placement in accordance with established level of care and patient aggregation criteria.

  • Manages the inpatient flow of patients from preadmission and admission, through the time of discharge and efficiently optimizes the utilization of inpatient bedsdentifies demand and capacity mismatch for inpatient beds.
  • Works in conjunction with referring physicians and nurses, accepting physician and nurses, and other interdisciplinary team members to assess, plan, and facilitate inpatient transfers.
  • Provides clinical screening on all admission requests using Interqual Criteria for medical necessity.
  • Screening is done for appropriateness and pre-certification.
  • Collaborates with the staff in utilization mnagement for follow up on urgent and emergent transfers and admissions.
  • Documents communication note in electronic medical record, with detailed inpatient, clinical information including reason for transfer and details of transfer plan.
  • Facilitates and coordinates the details of admissions with departments.
  • Coordinates inpatient bed assignments based on acuity, aggregation and bed availability.
  • Communicates to appropriate attending physician for issue resolution or intervention.
  • Monitors process of admission and mitigates any delays.
  • Identifies any quality management issues and reports to appropriate manager for follow-up.
  • Serves as a resource to physicians and hospital personnel regarding system-wide, inpatient bed availability.
  • Directs and alters assignments in response to changes in capacity and acuity of department.
  • Participates in coverage and on call systems per department
