About the role

The HMHC Assertive Community Treatment (ACT) Team serves individuals diagnosed with severe and persistent mental illness whose needs have not been met by traditional clinic-based services. The ACT team outreaches and enrolls people into its services, provides comprehensive mental health and social service support, helps to obtain and maintain housing, works with individuals to return to a full and satisfying life in the community. The ACT Team clinician provides services in the community and as a part of our ACT Team. This includes delivering services such as wellness self-management counseling, integrated dual disorder treatment, and assisting to improve daily living skills to consumers who have a history of poor engagement in traditional mental health services. This position works closely with other treatment teams as needed to provide services at a higher level of intensity or frequency with the goal of the consumer remaining in the community with resources in place.

Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

ESSENTIAL FUNCTIONS:

  • Participates in daily team meetings to assign cases to team members and review day’s work responsibilities
  • Clinician completes a comprehensive assessment to anyone entering the ACT Team and identifies services consumer should received in ACT based on the assessment
  • Utilizes identified and accepted methods to remediate symptoms of mental illness and/or improve emotional and functional well-being through trauma focused and person-centered evidenced based practices.
  • Works with ACT Team to develop a crisis prevention plan and treatment goals by working with the consumer identifying their needs to provide person-centered treatment
  • Participates in care coordination to collaborate with community partners, caregivers, family members, physicians and other treatment providers regularly
  • Support consumers to achieve treatment goals, coordinate with collateral contacts, medication education, crisis management, prompt client to complete daily living activities, monitor client one to one for safety or as needed
  • Completes ongoing diagnostic evaluation of consumers and completes required clinical assessments per organization policy and procedure
  • Participates in discharge planning and support transition to the community or to higher level of treatment.
  • Collaborates with team in plans to ensure clients attend mental health appointments
  • Contribute to a program culture that is person-centered, outcome-oriented and dedicated to continuous quality improvement
  • Schedule is varied based on consumer needs but will involve some evening, one weekend day and eventually on call rotating coverage to provide support so consumer assigned to the ACT Team at their time of crisis need
  • The majority of treatment is provided in the community where the consumer feels most comfortable
  • Provide culturally competent services in accordance with policies and procedures
  • Complete all required documentation and training in a timely and accurate manner as defined by the agency.
  • Other activities as assigned

MINIMUM KNOWLEDGE AND SKILLS REQUIRED:

  • Familiarity with community mental health services.
