About the role
Essential Functions:
- Provide primary medical care to palliative medicine patients and perform on call duties as assigned.
- Oversee nurse practitioners in the palliative medicine department both inpatient and outpatient, to ensure the delivery of high quality-evidence-based care. Work in conjunction with the Chief Medical Officer to provide guidance and support to providers in clinical decision making, complex case management and professional development.
- Provide leadership, guidance, and support to the clinical staff which includes nurses, Nurse Practitioners and social workers .
- Design and/or implement and review clinical programs that address patient needs and improve healthcare outcomes.
- Work in collaboration with the Practice Management and Hospice Management departments to implement quality improvement initiatives and optimize patient care coordination across multiple providers and settings.
- Monitor key performance indicators related to patient volume, satisfaction, and clinical outcomes.
- Facilitate effective communication and collaboration between healthcare teams, departments, and external stakeholders.
- Ensure compliance with relevant local and regional healthcare regulations, standards, and policies.
- Perform other related duties and responsibilities as required.
