About the role

Essential Functions:

  1. Provide primary medical care to palliative medicine patients and perform on call duties as assigned.
  2. Oversee nurse practitioners in the palliative medicine department both inpatient and outpatient, to ensure the delivery of high quality-evidence-based care. Work in conjunction with the Chief Medical Officer to provide guidance and support to providers in clinical decision making, complex case management and professional development.
  3. Provide leadership, guidance, and support to the clinical staff which includes nurses, Nurse Practitioners and social workers .
  4. Design and/or implement and review clinical programs that address patient needs and improve healthcare outcomes.
  5. Work in collaboration with the Practice Management and Hospice Management departments to implement quality improvement initiatives and optimize patient care coordination across multiple providers and settings.
  6. Monitor key performance indicators related to patient volume, satisfaction, and clinical outcomes.
  7. Facilitate effective communication and collaboration between healthcare teams, departments, and external stakeholders.
  8. Ensure compliance with relevant local and regional healthcare regulations, standards, and policies.
  9. Perform other related duties and responsibilities as required.
