This Certified School Nurse must report in person to the West Chester location. 

 

Job Purpose:

The CSN is responsible for implementing the Pennsylvania Leadership Charter School’s health program as defined by the Public School Code of 1949 (section 14), PA Code 28 (Chapter 23- School Health regulations). This position will oversee and maintain state records and reporting and is responsible for coordinating and scheduling health department staff for all PA Leadership locations.

 

Essential Duties and Responsibilities:

• Review and input immunization records for all new students (approx. 1,400 each year).
• Follow-up to request missing immunization and other health records, including annual notices re upcoming immunizations needed.
• Conduct health screenings at new student orientations.
• Maintain and update individual school health records for all students, including electronic and hard-copy file storage.
• Communicate to all parents re: required annual health screenings.
• Document diagnostic and annual health screenings in SNAP or similar health records information management system.
• Develop Plan of Care for students with food allergies, seizure disorders, asthma, or other health conditions for onsite programs, standardized testing, field trips, and other points of contact with students.
• Administer medication to students as requested by parents or physicians according to the medication guidelines.
• Ensure that emergency information for students and staff is maintained and that student and staff visits are logged.
• Act as a primary referral source when needed for all students visiting the health room.
• Use assessment skills to identify students at risk for physical and psychosocial problems, and communicate these health needs to parent or other school personnel.
• Provide for safe storage of medications, and reliable administration and recording of all doses given.
• Articulate the health needs of children to support parents, and assist families to utilize community resources to meet their children’s health needs.
• Provide health information and input as requested in the development of 504 and IEP plans.
• Develop IHPs (Individualized Health Plans) for students as necessary.
• Serve as a key member of the school Wellness Committee.
• Serve as a key member of the school Safety Committee.
• Participate in Student Assistance Program (SAP) and CARE team at PALCS.
• Provide referrals/resource information to families without access to health care.
• Order and maintain health and medical equipment and supplies at PALCS facilities, including portable first-aid and triage kits.
• Coordinate physician and dentist annual onsite visits to assist families needing health screenings.
• Promote good safety practices in school buildings and on school property. 
• Ensure the timely delivery of all state-mandated health reports as required by PDE and the Department of Health, including compilation of student health data.
