Job Description:
Clinical Pharmacist IThe Clinical Pharmacist is responsible and accountable for the provision of a broad range of inpatient and outpatient pharmaceutical care services to ensure safe, effective, and prompt medication therapy. Through various assignments within the department, this pharmacist provides support of centralized and decentralized services and delivers optimal medication therapy to patients with a broad range of disease states. The Clinical Pharmacist works in collaboration with multidisciplinary care providers to monitor, assess and provide clinical interventions to ensure medication use is aligned with patient care needs, evidence-based practices, established policies and protocols, and regulatory standards.EXPECTATION FOR ALL EMPLOYEES

POSITION SUMMARY
The Clinical Pharmacist is responsible and accountable for the provision of a broad range of inpatient and outpatient pharmaceutical care services to ensure safe, effective, and prompt medication therapy. The Clinical Pharmacist works in collaboration with multidisciplinary care providers to monitor, assess and provide clinical interventions to ensure medication use is aligned with patient care needs, evidence-based practices, established policies and protocols, and regulatory standards.
PRINCIPLE DUTIES AND RESPONSIBILITIES
- Performs all job duties and functions within the scope of practice for a Registered Pharmacist, as defined by the Oregon State Board of Pharmacy
- Engages in the practice of clinical pharmacy by providing patient care to optimize medication therapy and to promote disease prevention and patient’s health and wellness
- Performs drug utilization review and determines the presence of medication-therapy problems by evaluation of medication orders in light of the information contained in the patient’s record and consultation with the prescriber when appropriate
- Assesses and monitors individual patient drug therapy laboratory tests according to the established clinical policies and procedures and communicates with prescribers or other relevant healthcare practitioners
- Completes critical patient monitoring and reviews patient profiles to identify, prevent or mitigate medication therapy problems (for example: improper drug or dose selections, untreated indications, medications without indications, therapeutic duplications, treatment failures, drug interactions, adverse drug events and reactions)
- Provides clinical consultation and clarification to practitioners; recommends evidence-based medication therapy regimens and monitoring plans; and suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed
- Participates in the medication reconciliation process that includes verification of medication history lists obtained by pharmacy technicians, identification and resolution of medication discrepancies, communication with patients and Providers, and documentation of interventions
- Counsels and provides drug information to patients and their families
- Maintains proficiency in and actively engages in pharmacy operations and medication dispensing
- Provides oversight and ongoing supervision for all activities performed by supportive personnel (i.e. pharmacy technicians and interns) to ensure safe and accurate drug preparation, distribution and drug control
