As a Ambulatory Clinical Pharmacist at BUMC Tucson, you will support an outpatient Ambulatory pharmacy clinic focused mostly on anticoagulation. You will be working alongside another part time Pharmacist for full-time coverage.  You will provide in-person, direct patient care and include some remote management and interdisciplinary coordination. You will also be responsible for independently managing, initiating, modifying and discontinuing pharmacotherapy for multiple collaborative practices including anticoagulation, diabetes, hypertension and hyperlipidemia, most current referrals are for warfarin management. This part time position will have an average of 20 hours a week, 8AM - 4:30PM and the days of the week are flexible.

POSITION SUMMARY
This position actively participates on the healthcare team as a pharmacotherapy expert in the area of ambulatory care to provide high quality direct-patient care and serve as a resource and drug information specialist to all other healthcare staff. Contributes to the optimal selection of drug therapy and design and may implement patient-specific medication regimens to maximize therapeutic benefit, minimize toxicity and attain quality measure gap closure. Accurately and completely documents patient assessment data and clinical interventions. May provide comprehensive patient education to improve medication compliance and promote patient self-management skills. Must possess clinical acumen and skills necessary to assess and interpret data reflective of the patient’s clinical condition. Conducts educational activities, applies principles of performance improvement, and may participate in Committee activities as appropriate assisting with the development and implementation of evidence-based medication guidelines/protocols. May oversee residents, interns, pharmacy technicians and support staff as applicable, providing direction in completion of daily responsibilities.

CORE FUNCTIONS
1. Uses best practices and knowledge of internal or external business challenges to suggest improvements to services, processes or products. May provide oversight and assist with the development and implementation of evidence-based collaborative practice protocols. May participate in Ambulatory Pharmacy and Therapeutic (P&T) or other system committee activities as appropriate. Evaluates and recommends medications for formulary. Prepares formulary evaluations for P&T consideration.

2. Solves complex problems. Takes a new perspective using existing solutions. Performs medication therapy and disease state management, which includes critically evaluating disease states and adjusting medication therapy, per the ambulatory pharmacy collaborative practice protocols, to optimize patient regimens and improve clinical outcomes. Ensures utilization of appropriate and cost-effective medication therapy. Areas of focus may include, but are not limited to, disease state management (e.g. thromboembolic disease, diabetes, cardiovascular disease, COPD), medication management, adverse drug event prevention and management, and evaluating and addressing quality measure therapy gaps
