Sr. Diabetes Health Educator – Virginia

The Educator will provide diabetes education to our Virginia Medicaid population.  We are looking for a diabetes education who has experience working one on one with patients/clients/members providing education, assistance, and support. 

Required 

BC-ADM - Board Cert Advanced Diabetes Management

RD- Registered Dietitian (Required)

CDCES-Certified Diabetes Care & Education Specialist (Required)

Travel (Up to 25%)

May do face to face meetings in members home if needed.

Monthly Diabetic Clinic seminars at our Molina Location in Petersburg Virginia

Home office with internet connectivity of high speed required. 

Monday – Friday 8 AM to 5 PM EST (no weekend or holidays) 

 

Job Description 
Job Summary
Molina's Quality Improvement function oversees, plans, and implements new and existing healthcare quality improvement initiatives and education programs; ensures maintenance of programs for members in accordance with prescribed quality standards; conducts data collection, reporting and monitoring for key performance measurement activities; and provides direction and implementation of NCQA accreditation surveys and federal/state QI compliance activities.
  Knowledge/Skills/Abilities 
Develops, implements and evaluates health education programs for members within the Molina network and the broader community.  Provides oversight and assistance to providers in meeting health education contract requirements.  Additionally, functions in a lead role over other Health Educators and makes administrative decisions as delegated by the Manager or Director.
• Serves as a resource and lead to Health Educators in the resolution of issues that may arise during the implementation of Quality Improvement (QI) interventions. 
• Identifies, designs, implements, and evaluates health education interventions to meet the needs of the targeted population based on priorities established by the department and current contracts.
• Coordinates the health education services assessment of IPAs and medical groups as required and collaborates with other QI staff on oversight.
• Conducts population-, geographic- and member-specific needs assessments.
• Participates in the development of internal resources, in collaboration with others as identified within the annual work plan.
• Functions as a liaison to internal departments, community-based organizations, social service agencies and public health departments as assigned to ensure that Molina resource information is current and available.
• Provides input on the design and functionality of all utilized databases. .
  Job Qualifications



Required Education
Bachelor of Science in Health Education, Health Promotion or Public Health.
OR
Associate's Degree in health-related area plus a certification of specialization in patient education (e.g., Certified Diabetes Educator).                                                   
Required Experience
• Min. 4 years experience in Health Education.
• At least 1 year experience working with culturally diverse and low-income populations within a managed care setting.
• Previous supervisory or other leadership experience required.

Master's Degree in relevant field.
Preferred Experience
Knowledge of the Medicaid population.
Preferred License, Certification, Association
Certified Health Education Specialist (CHES)
