Milton Nursing & Rehab Center is seeking a PRN Physical Therapist (PT) to join our in-house rehab team in a skilled nursing/long-term care setting.

We are looking for a compassionate and flexible clinician who enjoys helping residents improve mobility, independence, and overall quality of life. This is a great opportunity for a PT seeking flexible scheduling while working alongside a supportive interdisciplinary rehab team.

Responsibilities

• Evaluate and treat residents in a SNF/LTC setting

• Develop individualized treatment plans and functional goals

• Collaborate with therapy, nursing, and interdisciplinary teams

• Complete timely and accurate documentation using Net Health and PCC

• Support resident safety, mobility, and independence

Qualifications

• Current Pennsylvania PT license

• SNF/LTC experience preferred, but not required

• Strong communication and clinical skills

• Flexible and dependable

• Compassionate, patient-centered approach to care
