Location/Schedule:
This role offers a hybrid schedule, with four days on-site at pediatric primary care practices in Charlestown, Revere, and Boston (165 Cambridge Street) and one remote day per week. You will also attend bi-monthly meetings in Assembly Row, Somerville and may meet families in their homes, schools, or public spaces as needed.
In addition, during the initial first 30 days of training there will be on-site training in other practices throughout the region (subject to mileage reimbursement if appropriate).
Job Summary
In collaboration with the RN/SW Care Manager, the Community Health -Behavioral Health Worker will assist families with gaps in care by assisting patients and families to access state and community services such as Mass Health, DDS, and SSI, as well as helping with navigation of these state systems.Locate and coordinate in home services such as ABA and other supportive in-home services. Follow up with families on status of applications.
The Community Health -Behavioral Health Worker may attend meetings with MH/BH community, pediatric specialists, and intensive care coordination services with the state, school, and community agencies. The Community Health -Behavioral Health Worker will assist families to identify gaps in resources while providing necessary support to the patients and families with mental and behavioral health diagnoses.
In collaboration with iCMP RN/SW, the Community Health -Behavioral Health worker will:
•Involve participant in self and community advocacy
•Map communities to help locate support services (i.e.: Housing, food, shelter, community services, etc.). Use existing institutional and community resources and maps to achieve this and educate other team members on resources found.
•Connect families with needed resources, including tracking referrals when appropriate.
•Assist patients and families in applying for State resources and services (i.e. Mass Health, DTA, DDS, and DMH). This may include helping them gather necessary documentation, filling out forms, and helping them anticipate and prepare for re-applications when needed. Follow up with families on status of applications.
•Locate and coordinate services for children with Autism (i.e. ABA therapy, social skills groups, camps, in-home supports).
•Locate mental health providers within a patient’s insurance network and collaborate with PCP and iCMP Team on ensuring referrals to appropriate providers.
•Attend meetings regarding educational and mental health services for patient. Support families in advocating for services. Attend 504/IEP meetings with families. Coordinate with iCMP Care Team and PCP meeting details and next steps.
Qualifications
- High school diploma or equivalent required. Associates/Bachelors degree in related field preferred.
- 1-2 years prior experience working with children and families dealing with complex medical and psychiatric/behavioral problems.
