Job Summary: As part of a grant-funded initiative, the Patient Experience and Community Health Advisor is responsible for supporting the Director in achieving the goals and initiatives to improve patient engagement, experience, and satisfaction, including all the diverse communities the hospital serves. Supports patient needs regarding social drivers of health, offers community resources and referrals, provides navigation, and support to meet those needs. Works collaboratively with the Patient Experience team to promote patient-centered care and actively participates in multidisciplinary patient-centered team meetings. Coordinates the hospital's patient complaint and grievance processes in accordance with the Centers for Medicare and Medicaid and other regulatory agencies' requirements. Coordinates the hospital's patient satisfaction survey process. Works with department managers and senior leaders to analyze patient feedback data from multiple sources to identify improvement opportunities and develop associated action plans.
Specifications: Bachelors degree in related field required, Masters degree in related field preferred. At least 1-3 years related experience required; demonstrated subject matter expert on current patient experience. Body of Knowledge; demonstrated leadership with management experience preferred.Strong interpersonal skills to drive change, manage challenging situations, and to interact with all levels of professionals as well as patients, families, guests and the community required. Must have strong communication, teaching, facilitation, project management, and leadership skills. Public speaking, presentation skills and proficiency in Microsoft Office required.
