In this role, you will manage and support the agency’s suicide care pathway, conduct follow-ups with clients, collaborate closely with clinical teams and community partners, and ensure accurate tracking and documentation of services. You will also contribute to suicide prevention education and participate in crisis response efforts, helping to ensure that clients receive timely, coordinated, and effective care.

Key Responsibilities:
• Coordinate and manage the agency’s suicide care pathway, including conducting timely and supportive follow-up calls with clients at risk
• Provide compassionate crisis intervention services via phone and face-to-face to support individuals experiencing suicidality
• Collaborate with treatment teams, hospitals, law enforcement, and community partners to ensure continuity of care and client safety
• Connect with service providers regarding safety planning and assist in coordinating appropriate interventions
• Educate staff and community stakeholders on suicide prevention, including risk factors, warning signs, and intervention strategies
• Monitor, track, and analyze data related to suicidality outcomes and ensure accurate reporting in the electronic health record (EHR)
• Participate in Zero Suicide Committee meetings and Ad-Hoc Risk Management/Death Reviews to support continuous quality improvement
• Work closely with Quality Management to evaluate outcomes and enhance prevention strategies across client populations
