Senior Care Management Coordinator

Senior Care Management Coordinator at Providence Portland Medical Center, in Portland, OR.

Various Shifts and Schedules Available include a Full-Time/Day and a Per Diem/Variable 

This position coordinates the care transitions of the patient through assessment, planning, implementation and evaluation of appropriate services and resources to maximize optimum outcomes. This position works collaboratively with the multiple disciplines, both internal and external to the organization, in effecting quality and cost-effective management of the patient's care along the continuum.

Required qualifications:

• Bachelor's Degree in Healthcare related field or 4 years care management experience
• Coursework/Training and Completion of LPN/LVN program or equivalent education/experience
• 3 years of Healthcare Experience, or 2 years of LPN (for equivalency to education and healthcare experience)

Preferred qualifications:

• 3 years of Discharge Planning or Care Coordination

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
