Key Responsibilities Conduct thorough assessments and evaluations of patients with various functional limitations and disabilities, developing individualized treatment plans that address their specific needs. Consult with healthcare providers to ensure the optimal continuum of care for each patient throughout their rehabilitation journey. Provide detailed documentation of evaluations, treatment plans, progress notes, and discharge summaries while adhering to all relevant regulations and documentation standards. Implement treatment plans that incorporate evidence-based practices, carefully monitoring patient progress and making necessary adjustments. Collaborate with interdisciplinary team members, including physicians, nursing staff, and support personnel, ensuring comprehensive patient care. Engage in community outreach initiatives, contributing to program development, marketing efforts, and quality improvement projects. Supervise support staff efficiently, ensuring they follow best practices while encouraging professional growth and development. Continuously maintain professional competencies through self-directed learning, attending workshops, and pursuing continuing education opportunities. Patient Population / Client Focus In this role, you will work primarily with patients recovering from orthopedic injuries, neurological disorders, and post-operative rehabilitation needs. Your compassionate approach will be crucial as you guide patients of all ages and backgrounds through their recovery journeys, fostering engagement with treatments and empowering them to reclaim their independence. Qualifications A Bachelor’s Degree in Physical Therapy from an accredited institution is required. State Licensure in Physical Therapy is mandatory prior to commencement of work. BLS (Basic Life Support) certification is essential. Exceptional verbal and written communication skills are necessary to facilitate patient education and interdisciplinary collaboration. Proficiency in technology, including electronic medical records (EMR) systems, is required to enhance patient documentation efficiency. Preferred Experience Experience in a rehabilitation setting is preferred, particularly with diverse patient populations. Previous involvement in community service or outreach programs will be advantageous. Familiarity with electronic documentation systems and rehabilitation software will enhance your effectiveness in this role.
