Home Health RN Clinical Supervisor -Hybrid

Inspired Homecare is looking for a Home Health Clinical Supervisor, to oversee all patient care and clinical personnel, ensuring care is coordinated, compliant, timely, and aligned with individualized plans of care and agency standards.

Responsibilities:

* Supervise all patient care services and staff (RN, LPN, OT, PT, HHA)

* Coordinate patient care and referrals

* Ensure ongoing assessment of patient needs

* Provide direct patient care when staffing requires

* Support clinicians with OASIS, documentation and case management 

* Review clinical documentation

* Lead case conference, clinical meetings, and in-services

* Ensure patient charts and records are accurate, and regulatory compliant

* Monitor clinical outcomes

Qualifications:

* Registered Nurse (RN) with current state license

* 2-3 years of home health experience required

* Strong leadership, communication, and problem-solving skills

* CPR Certification, valid driver's license and auto insurance

Sign on Bonus: $1250, after the first 90 days, and another $1250 after a year of employment
