What You’ll Do:
🔹 Oversee and coordinate all social services for residents
🔹 Support admissions, discharge planning, and community outreach
🔹 Advocate for resident rights and quality of life
🔹 Lead family meetings and provide emotional support
🔹 Ensure regulatory compliance and accurate documentation
🔹 Collaborate with interdisciplinary teams to support holistic care

What We’re Looking For:
✔️ Previous experience in social services (SNF experience a big plus!)
✔️ Strong leadership, communication, and organizational skills
