Job Summary:
The LPN or RN Nurse Case Manager is responsible for overseeing the delivery of skilled nursing care to patients in their place of residence, ensuring high-quality care is provided on either an intermittent or ongoing basis. This role involves collaborating with the client, their family, physicians, and the multidisciplinary healthcare team to develop and manage personalized care plans. The Clinical Supervisor will assess patient needs, coordinate care, and manage a caseload of new and existing clients. All services are provided under the guidance and supervision of the Program Coordinator.
Requirements
Home Health Services:
1. Evaluates the patient’s nursing needs.
2. Coordinates with marketing to contact family/pcp for HH order.
3. Oversees client upon discharge of HH for further need.
4. Perform other duties as assigned.
Private Duty / Facility Services:
1. Interview client and their families in facility for need.
2. Provide necessary information for client / family about private duty and package services.
3. Perform initial nursing evaluation / assessment visit, document assessment information utilizing
approved procedures and forms.
4. Identify high risk factors, barriers to goals, and strengths that will assist the client in
achieving goals.
5. Develop a comprehensive plan of care that addresses all pertinent areas of the assessment
6. Implement services as directed by plan of care.
7. Provide frequent, timely reviews of the plan of care with the client and re-evaluates and modifies
the plan based on client outcomes.
8. Assure quality of services by gathering information from direct observation, telephone or in
writing all resources involved in the client plan of care.
9. Any changes in the plan of care are appropriately communicated.
10. Directly staff and supervise all facility based staff and supervise any field staff in facility.
11. Update and communicate with all members of the private duty / facility team.
12. Take on-call rotation as determined within the office/management staff, handling all staffing and
nursing issues that arise.
13. Perform other duties as assigned.
