ABOUT THE ROLE

As Manager, Population Health Engagement (Quality Outreach), you’ll have the opportunity to directly solve some of healthcare’s most demanding challenges. You will lead a growing team by creating a new central Pop Health outreach function. This team of specialists will be focused on engaging and partnering with patients to encourage healthy actions through empathetic and motivating communications that help them address Quality gaps in care. You will design and manage gap closure processes and workflows to ensure reliable, coordinated, closed-loop holes across relevant systems (e.g., Salesforce, EMR, phone, and medical records). Reporting to the VP of Pop Health, you’ll be responsible for driving team, company, and patient outcomes in partnership with team members across Galileo. 

Here’s what you’ll do:

  • Manage a growing team of Population Health Engagement Specialists (Quality Outreach Representatives)
    • Manage a team of specialists to execute telephonic & other outreach to Galileo patient populations in support of Quality gap closure across Healthcare Effectiveness Data and Information Set (HEDIS) and other payor or Galileo standard of care Quality measures and outreach efforts (e.g., post-discharge targeted engagement)
    • Grow and shape the team to be high-performance and high-impact, with rapid learning cycles
    • Oversee day-to-day operations of the team
    • Foster a positive and collaborative culture
  • Design and deploy effective end-to-end operating processes & workflows for the team
    • Develop a thorough understanding of different populations under management by contract and corresponding Quality targets
    • Engage cross-functionally to shape streamlined information flows and tools and create closed-loop processes that enable specialists to close gaps and achieve critical pop health outcomes and targets, maximizing effectiveness and improving team experience
    • Build out workflows tailored to systems (e.g., Salesforce, EMR, Dialpad, external medical record portals), measures, and populations as appropriate
    • Refine outreach messaging in collaboration with Marketing and coach the team on communication and motivational interviewing to continuously improve the effectiveness of patient engagement
    • Systematize performance reporting / KPIs, training, documentation, and tools for direct reports to meet or exceed gap closure goals successfully
  • Serve as a key leader within the Pop Health team, supporting the achievement of Galileo Pop Health targets
    • Translate performance goals/targets into caseload for the specialists
    • Contribute to strategy and work-planning discussions with Pop Health leadership
    • Support team members in relentlessly identifying and creatively problem-solving barriers to closing gaps in care through clinical or other action (e.g., documentation, exclusions)
    • Identify and pursue opportunities to partner with Galileo teammates and external organizations/resources to improve outcomes

 

ABOUT YOU

As the Manager, Population Health Engagement, you bring Galileo a blend of people management, process design & implementation, and related healthcare experience. To be successful, you must be driven and accountable. You demonstrate a commitment to results, precise measures of performance, and quick iteration for ongoing improvement. You are comfortable with an evolving product and organization, managing ambiguity and complexity without distracting from the mission, and can thrive in a fast-paced start-up environment. You have the business savvy to engage a diverse consumer base, including seniors, developing messaging, targeting, and tone to compel patients to take action on health goals in a way that resonates.
