POSITION SUMMARY: Reports to the Program Coordinator(s) of the Youth Care Management program. In addition to providing Care Coordination services to an abridged caseload, the Senior Youth Care Coordinator works as part of the administrative team to oversee the operations of the Youth Care Management program. The Senior Youth Care Coordinator is responsible for implementing and maintaining policies and procedures, maintaining operations and clinical functions of the program as well as directly supervising a team of Youth Care Coordinators.
ESSENTIAL ACCOUNTABILITIES:
1. Maintain all duties and responsibilities of a Youth Care Coordinator, including managing a modified caseload (see YCC Job Description).
2. Supervise a team of 3 - 5 Youth Care Coordinators.
3. Review submission of all documentation in client charts (including progress notes, intake assessments, annual updates and discharge documents) for all team members, including timeliness of submission as per health home deadlines as well as for content and quality of services provided.
4. Maintain a working knowledge of team caseloads, Health Home policies and procedures and Health Home systems and platforms.
5. Train new staff members in all areas of program policy, health home requirements/deadlines and submission of documents into the EMR.
6. Attend intakes/home visits with team members as needed to ensure staff safety, provide guidance for new staff, or to offer consultation as requested.
7. Act as consultant for all members of the team to provide information regarding community resources, social welfare programs and other information as needed.
8. Address client concerns and/or grievances that are not able to be resolved directly with the Youth Care Coordinator.
9. Attend and participate in administrative meetings as scheduled.
10. Attend individual supervision as scheduled with the Program Coordinator(s) to discuss needs and concerns and work on supervisory skills development.
11. Attend periodic trainings, webinars and meetings to enhance skill levels and remain abreast of system changes.
12. Co-facilitate monthly Youth Care Management meetings to address agency policy updates, HHCM policy updates, paperwork, billing, trainings, resources, etc.

13. Obtain and maintain CANS-NY Assessment Certification (Supervisory staff must obtain an
80% or higher on certification test).
14. Implementation and maintenance of program policies and procedures.
15. Responsible for revenue monitoring and reporting.
16. Implementation and monitoring of Performance Improvement Plans.
17. Provide leadership with systemic and programmatic design.
18. Review of all referrals and eligibility determinations. Assure maintenance of eligibility
documentation.
19. Work closely with Program Coordinators to address any issues that arise with staff and to
ensure smooth implementation of program policy.
20. Be responsible for agency vehicles, including upkeep, documentation, and gas card when
assigned.
21. Be responsible for agency cell phone, laptop, and associated items.
22. Follow program guidelines as outlined in the personnel manual.
23. Report to the program administration any issues and/or concerns on a regular/as needed basis
while working in the field.
24. Responsible for having access to an insured, personal vehicle.
25. Track and submit mileage.
26. Assist Program Coordinator(s) with any additional tasks concerning departmental needs.
27. Other duties as assigned.
