Schedule: Full Time, Rotating Shift (Flat Rate)  Join Our Case Management Team!Are you a recent graduate or currently enrolled in a Master’s of Social Work (MSW) program? Indiana University Health is seeking an Intern to support patient transition and care coordination efforts. This position offers an excellent opportunity to gain hands-on experience in a multidisciplinary environment, advocating for patients and connecting them with essential community and financial resources.Position Overview:The Intern works as part of a case management team to provide client services and follow-up as needed. Your role involves assisting individuals experiencing personal and environmental challenges that may predispose them to illness or hinder their ability to benefit fully from medical care. You will advocate for patients by helping secure community and financial resources through your knowledge of various human resource agencies.This position is designed to support patient needs, promote care coordination, and enhance overall patient outcomes.Qualifications & Requirements:

• Education: Must be a graduate of an accredited MSW program, within 120 days post-graduation, or possess a Bachelor's degree in social work or a related field.
• Communication Skills: Strong written and verbal communication abilities.
• Knowledge & Skills:
  • Understanding of family dynamics, poverty, and diverse cultures.
  • Extensive knowledge of community resources and social service agencies.
  • Ability to function effectively in a multidisciplinary team setting.
