Cleveland Clinic Martin Health is looking to add a Homecare RN to the team who will support Maternal Child Outpatient Services. In this role, you will visit new mothers in their homes one week after delivery. Some of your responsibilities include conducting a full physical evaluation for both the mother and baby, including weight checks for the baby and a mental health evaluation for the mother. This position also entails providing patient education on home and infant safety. You will typically visit an average of 6 patients per day, with the possibility of up to 8, all within Martin and St. Lucie counties. Delivering individual, high-quality care for the mother and baby can result in a positive patient experience and helps ensure the mother is receiving all continued services that are needed. 

 

A caregiver in this position works Monday-Friday from 8:00AM – 5:30PM. 

 

A caregiver who excels in this role will:  

  • Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization. 

  • Perform accurate and complete starts of care, resumptions, recertifications and routine visits. 

  • Perform medication reconciliation at each visit. 

  • Assess patient for safety risks, abuse and neglect. 

  • Serve as a primary coordinator of all disciplines for well-coordinated patient care. 

  • Assess, document and manage patients' pain. 

  • Prioritize and document patient problems based on assessed needs and problems in collaboration with patient, family and health care team. 

  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. 

  • Escalate change in patient condition to patient’s physician as appropriate.
