Essential Duties and Job Responsibilities

  •  Organize and maintain paper and/or electronic health records, ensuring they are complete, accurate, and easily accessible. 
  •  Follow facility and regulatory guidelines for medical records storage, retrieval, and disposal. 
  •  Enter and update resident information, including demographic data, diagnoses, procedures, and medications. 
  •  Retrieve medical records for medical staff or other authorized individuals. 
  •  Review records for completeness, accuracy, and timeliness, identifying and addressing discrepancies. 
  • May assist with data collection, analysis, and reporting for quality improvement or other facility purposes. 
  • Adhere to HIPAA regulations and maintain the confidentiality of patient information.
  • Ensure the accuracy, completeness, and timely completion of medical records, including completion of discharge sheets and 360 care documentation.
  • Maintain accurate records of all actions taken, including record releases, audits, and system updates.
  • Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator.
  • Must be knowledgeable on federal and state laws regarding medical records.
  • Review and audit admissions, discharges, change of condition, and other pertinent records on nursing units.
  • Ensure that incomplete records/charts are returned to nursing service for correction.
  • Ensure that registries are properly maintained for admission and discharge of residents.
  • Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the facility to assure that medical records can be properly maintained and problem areas can be identified and corrected.

Requirements and Qualifications

  • High school diploma or equivalent.
  • Basic understanding of medical terminology and anatomy/physiology.
  • Proficiency in Microsoft Office Suite and/or electronic health record (EHR) systems.
  • Experience in medical records or healthcare administration is preferred. 
  • Excellent communication and interpersonal skills.
  • Strong organizational and multitasking abilities.
  • The ability to work in a fast-paced, crisis-prone environment.
  • Advocacy skills with a focus on patient-centered care.
