What You’ll Do:

• Provide in-home visits supporting patients and families.

• Deliver counseling, care planning, and crisis intervention .

• Connect patients to community resources and support systems .

• Collaborate with an interdisciplinary care team .

What You Bring:

• Master’s Degree in Social Work (MSW) required.

• Active LGSW or LISW license in Minnesota .

• At least 1 year of clinical Social Work experience .

• A passion for independent, relationship-based care .
