Your Role
As a Pharmacy Care Coordinator with Evergreen Nephrology, you play a critical patient-facing and operational role in supporting our pharmacy care management programs. You will work closely with pharmacists and other care team members to engage patients and ensure timely access to medications and pharmacist-led clinical services. This role is focused on outbound patient outreach, care coordination, and navigating medication barriers for patients living with chronic kidney disease and other complex chronic conditions.
You are often the first voice patients hear from the pharmacy care team, helping establish trust, explain the value of pharmacy services, and remove barriers that prevent patients from staying consistent with their medications. The Pharmacy Care Coordinator schedules pharmacist consultations and coordinates across pharmacies, prescribers, and external programs to ensure patients receive the right medications at the right time.
While this position is fully remote, you must be able to work 8:30am – 5pm in the central time zone.
Role Responsibilities
- Conduct outbound telephone outreach to patients to introduce Evergreen’s pharmacy care management services, establish rapport and trust, and engage patients in pharmacist-led consultations.
- Schedule pharmacist consultations and coordinate follow-up, ensuring appropriate continuity of care between patients, care management pharmacists, and other members of the care team.
- Serve as a liaison between patients, community and mail-order pharmacies, and provider offices to resolve medication barriers, including refill coordination, adherence gaps, or social determinants of health concerns.
- Coordinate referrals to enhanced service pharmacy partners, including pharmacies offering adherence packaging (e.g., pill packing or blister packaging), and support onboarding and transition processes.
- Assist patients with enrollment and ongoing participation in medication assistance programs, including manufacturer patient assistance programs, FQHC pharmacy partnerships, or other access resources.
- Accurately document patient interactions, outreach attempts, care coordination activities, and outcomes in Evergreen’s clinical and operational systems.
- Collaborate closely with Care Management Pharmacists to support medication adherence initiatives, quality gap closure, and value-based care program goals.
- Maintain familiarity with common chronic disease medications, CKD-related therapies, and high-level insurance concepts to effectively communicate with patients and external partners.
- Uphold patient privacy and confidentiality standards in accordance with HIPAA and organizational policies.
- Other duties consistent with this role, as assigned.
Success in this role is measured by successful patient engagement, effective coordination with pharmacies and providers, timely scheduling of pharmacist consults, and other contributions to improved medication adherence and patient experience.
Required Qualifications
- High school diploma or equivalent
- Strong interpersonal and communication skills, with the ability to build trust and engage patients over the phone
- Experience in healthcare, pharmacy, care coordination, or other patient support
