About the role
- Communication
- Engaging and supporting patients in the management of disease.
- Engage in problem solving with clients to address adherence problems and barriers to care.
- Assist patients to adhere to care by assisting to make appointments and appointment reminders.
- Engage "harder-to-reach" clients who have missed appointments, fallen out of care, or have not entered care.
- Care Planning
- Completes and documents patient visit notes in the EMR accurately, as directed by organizational processes and practices and regulatory guidelines, within scope of practice.
- Understands and adheres to corporate compliance procedures, accrediting bodies standards and/or regulatory guidelines and requirements.
- Assist persons to navigate the service system and assist them to participate in care and services.
- Participate in care conferences with the multidisciplinary team regarding patient concerns.
- Assign patients to Care Coordinators for HLN programs.
- Responsible for workflow processes to support various network initiatives as assigned.
- Run designated reports.
- The Patient Care Coordinator 2 is prohibited from conducting evaluations or interpretations of clinical data.
- Other Duties
- Performs other duties as assigned or directed.
MINIMUM REQUIREMENTS:
Experience: 1 year experience in a clinical capacity and knowledge or experience with patients of assigned disease program and medical treatment.
Education: High school diploma or equivalent. Completion of a Medical Assistant program.
Skills and Training: Good communication skills.
