Job summary

The Specialist Palliative Care Team is an established fully integrated community multi-disciplinary team, embedded in Specialist and Community Services in Hampshire and Isle of Wight Healthcare NHS Foundation Trust.

The Specialist Palliative Care Service provides evidence-based support to adults with a life shortening diagnosis, for both non-malignant and malignant diseases. It is available at any point during a person's life shortening illness, not just at the end of their life, where their symptoms, physical or psychological, have not responded to routine therapeutic interventions. Following a palliative diagnosis, we aim to empower the patient, working with them and their care givers, to "Live well until they are dying", using a flexible holistic approach. Contacts and support utilise various mediums such as face to face visiting, remote on-line consultation and telephone support. It has clearly defined aims of service, referral pathways and discharge processes.

As part of Hampshire & Isle of Wight Healthcare the service covers Portsmouth, Fareham and Gosport and South Eastern Hampshire, with hubs split across the two localities. In addition, the service acts as an expert resource in Specialist Palliative Care for other generic services needing advice and guidance for care management and future care planning, through education and support.

The Nursing part of the Service operates an integrated Weekend and Bank Holiday cover.

Main duties, tasks & skills required

*To work autonomously and as part of the multidisciplinary team, with patients; their families and carers, co-ordinating organising and managing a defined caseload.

*Working in partnership with patients, carers, and service users, you will advise the teams with responsibility for providing the highest standards of patient care through on-going assessment, planning, implementation, and evaluation of programmes of care.

*Works autonomously to manage a complex caseload of patients, as part of the Specialist Palliative Care multi-disciplinary team.

*As part of the management of complex symptom control provide advice on the tailoring of pre prescribed medications in response to changing clinical need.

*Contribute to and participate in multidisciplinary working to ensure palliative care needs of patients are identified and met.

*Work as an Independent Prescriber to prescribe medication according to patient need adhering to the guidelines from the National Prescribing Centre, and the Hampshire and Isle of Wight Healthcare NHS Foundation Trust Policy.
