Temporary SOR Housing Supports Case Manager

This is a grant-funded position which may end at the conclusion of the grant. The starting pay rate for this position is $20.40. The Human Services Director's Office is currently accepting applications for a full-time, Temporary SOR Housing Supports Case Manager. The Housing Supports Case Manager works with individuals within Dauphin County who are experiencing housing difficulties, at-risk of homelessness, and/or who are experiencing homelessness to apply for and/or qualify for the State Opiate Recovery Program (SOR), and obtain affordable housing, and also apply for, obtain and retain Housing Vouchers or eligibility for subsidized housing as well as other mainstream programs. The Housing Supports Case Manager works actively to increase housing opportunities for people throughout Dauphin County. The Housing Supports Case Manager assists with assessing and approving SOR applications for individuals involved with contracted recovery houses, our Human Services departments, criminal justice system, or those returning to the community from any period of incarceration at Dauphin County Prison as well as involvement in group violence in the city of Harrisburg. In this context, the Housing Supports Case Manager will develop relationships with clients and/or re-entrants and ensure that their housing options are fully explored. This will include working with outside contacts to act as a liaison between the residents and the existing human services and supports. The Housing Supports Case Manager must be prepared to work with individuals with a medical, mental health concern, or substance use disorder. Responsibilities will include case management and assisting people who have difficulty finding homes, such as reentrants, those seeking recovery from SUD, those with medical concerns, victims of group violence, and individuals with a history of trauma. For these cases, the Housing Supports Case Manager coordinates with the community and other professionals to find suitable and affordable arrangements for people and their families.
MINIMUM EDUCATION, EXPERIENCE, AND TRAINING:
- Bachelor’s degree in social work or other human services, and two years of work experience or equivalent years of human services experience working with individuals with mental health, Autism, or developmental challenges and/or substance use disorders or criminal justice involvement.
OTHER REQUIREMENTS:
At the time of hire, a valid driver’s license and positive driving record, a favorable criminal background check, child abuse and FBI clearances, as well as favorable references are required. The applicant must provide proof of a valid Pennsylvania driver’s license within 60 days from the date of hire. An alternative form of identification will be accepted if you are not eligible to receive a Pennsylvania driver’s license under 67 PA. Code § 83 and driving is not an essential function of the position.
