WHAT YOU CAN EXPECT

• Modify outreach frequency in order to effectively monitor self- management to prevent adverse outcomes.

• Utilizes motivational and health-coaching techniques to engage the beneficiary and support system.

• Assist with coordinating services with Clinical Case Coordinator's direction, as needed (e.g., durable medical equipment, Home Health, etc.)

• Reinforces education to the beneficiary/patient and/or support system. Identifies gaps in care and in collaboration with CCC and/or provider assists with facilitation of resources, if appropriate to meet specific patient needs.

• Provides or facilitates appropriate education, using a beneficiary and family-centered approach, to facilitate the achievement of health goals.

• Collaborate with health care team (e.g., physician, nurses, pharmacist, etc.) and elevates treatment plan questions/concerns and interventions for further discussion with health care team on SDoH factors impacting patient adherence and success in meeting their health care goals.

• Assist with identification of educational needs of beneficiary/patient, support system and health care team.

QUALIFICATIONS

• Must be either a Licensed Practical Nurse, Medical Assistant or Pharmacy Technician

• High School Diploma/GED- Required

• If LPN- Must have graduated from accredited LPN Program

• Licensed Practical Nurse (LPN) State Licensing Board- Required

• If MA- Must have passed state accredited Medical Assistant Course

• Certified Clinical Medical Assistant (CCMA), National Healthcare Association (NHA), Registered Medical Assistant (RMA), Nationally Registered Certified Medical Assistant (NRCMA), Certified Medical Assistant (CMA), American Association of Medical Assistants (AAMA), National Certified Medical Assistant (NCMA) or National Center for Competency Testing (NCCT)- Required

• Pharmacy Tech- State Certification (PHT) or Pharmacy Tech Certification Board (PTCB)- Required

• Basic Life Support Program (BLS) - American Heart Association- Required

• 2 years of Experience in a Medical Office, Pharmacy, or Healthcare setting- Required

• 2 years of LPN, MA or Pharmacy Tech Experience- Preferred
