Care Navigator RN | Behavioral Health Observation Unit

Find your purpose being an integral part of the CentraCare hospital based behavioral health team as a Care Navigator Registered Nurse!  In this role you will work with the interdisciplinary team in facilitating patient care through collaboration, communication, facilitation, coordination, advocacy and resource management.  You will be accountable for a designated patient caseload and will plan care effectively to meet patient/family transitional needs, monitor length of stay, and promote efficient utilization of resources. 

Schedule

• Full-time | 80 hours every two weeks
• Monday – Friday | 10-hour day shift | 7am-5:30pm
• St. Cloud Hospital | Behavioral Health Observation Unit

Pay and Benefits

• Starting pay begins at $41.34 per hour; exact wage determined by years of related experience.
  • Pay range: $41.34 - $62.01 per hour

Qualifications

• Bachelors or higher degree in nursing required. 
• Current licensure as a registered nurse in the State of Minnesota required.
• 2 years minimum clinical experience and/or case management experience as a registered nurse required. 
• Knowledge of at least the following areas: abuse issues, chemical dependency, adult/adolescent/child clinical assessments. 
• Knowledge of crisis theory and intervention, psychiatric diagnosis, psychotropic medications, including use and side effects and knowledge of community resources and intervention.
• American Nurses Association (ANA) Psychiatric – Mental Health Certification preferred.
• Nonviolent Crisis Intervention (NCI) initial and on-going training required and provided.
