  • Monday through Friday
  • Day shift, no weekends, holidays, or on-call responsibilities

Position Summary:

The Pulmonary Nurse Navigator supports the Lung Nodule Program by coordinating patient care, facilitating communication between patients and providers, and ensuring timely diagnosis and management of lung nodules. This role involves triaging referrals, scheduling diagnostic procedures, providing patient education, and participating in multidisciplinary care planning to optimize patient outcomes.

Key Responsibilities:

  • Referral Triage & Coordination: 
    • Review and prioritize referrals from the Emergency Department and primary care providers.
    • Review Cerner excel inquiries for incidental lung nodules on CT scans.
    • Communicate with ordering providers, send educational brochures, and facilitate referral requests as needed.
  • Patient Scheduling & Follow-up: 
    • Schedule follow-up tests such as CT scans, PET scans, IR biopsies, or ion navigation bronchoscopy.
    • Reiterate care plans and next steps with patients after provider visits.
    • Ensure appropriate follow-up with oncology, radiation, and thoracic surgery teams.
  • Patient Communication & Support: 
    • Contact patients with test results, explaining findings and further recommendations.
    • Serve as the primary contact for patient questions, concerns, and feedback, and relay information to providers.
  • Care Coordination & Multidisciplinary Collaboration: 
    • Participate in multidisciplinary conferences to coordinate patient care.
    • Develop individualized care plans partnering with patients and primary physicians.
    • Collaborate with physicians, nurses, social workers, and other team members to ensure timely testing and treatment delivery.
  • Patient Advocacy & Education: 
    • Advocate for patient needs and preferences throughout their care journey.
    • Provide education about lung nodules, follow-up procedures, and management plans.

Qualifications:

  • Requires an Associates of Nursing (ASN).
  • Bachelors of Nursing (BSN) preferred.
  • Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
  • Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.
  • Requires Basic Life Support certification through the AHA.
  • Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
  • Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.
  • Requires knowledge of personal computers using various types of word processing and database software.
  • Requires clinical work experience, medical terminology.
  • Requires understanding of medical record requirements, regulations and policies.
