Diagnostic Imaging Patient Navigator

Job Description

Diagnostic Imaging Patient Navigator guides patients through the healthcare system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. The primary role will be with breast health patients, but will have responsibilities to assist with other screening programs.

Department: Diagnostic Imaging

Hours: Monday-Friday, 8am-4:30pm

Requirements: IEMA, ARRT, BLS, CBEC

Pay based on applicable experience; starting at $25.08

Responsibilities

Acts as a patient advocate as patients move through diagnosis and initial follow-up care. , Assesses patient prior to discharge and gives instructions for follow-up care. , Assists with  lung screening program with such things as checking orders for accuracy, appropriateness, and completeness, reviewing  shared decision making visit for completeness., Assist with breast biopsy procedures, Communicates with referring physicians and/or radiologists regarding pathology results and attains post pathology result recommendations. , Coordinates scheduling of US and stereotactic biopsies, Enters the radiologist’s recommended follow-up codes for breast and lung screening into the tracking systems., Follow-up lung rad 3, 4 and 5. , Follow-up with bi-rad 3 (short –term follow-up) patients to schedule and obtain physician orders. , Follow-up with patients post procedure. , Other duties as assigned, Provide information, comfort, support and education to patients and families. , Provide information about internal and external resources to assist patients with financial and emotional support., Provides community education regarding screening programs., Record pathology and other follow-up results in the mammography tracking system., Schedules patients with radiologist recommendations of bi-rad 0, 4, 5 for follow-up care.

AA (Required), AS (Required)
