Description

As a Clinical Pharmacist, you will facilitate medication management through medication reconciliation, evaluation of adherence, effectiveness, and toxicity, and you will act as a resource for primary care providers and care team members regarding medication to improve drug prescription usage and therapeutic outcomes. This position will work closely with the clinical staff and will report to the Director of Pharmacy.

JOB RESPONSIBILITIES:

  • Conducts and evaluates medication histories, assesses compliance and suggests modifications for optimal drug product selection in order to achieve desired outcomes; instructs patients in correct use of prescribed medications.
  • Individualizes medication regimens using sound principles and understanding of drug absorption, distribution, metabolism and elimination to establish and improve to optimum patient benefit.
  • Prevents, detects and resolves medication-related problem. Makes evidence-based, patient-centered drug therapy recommendations as part of the Integrated Care Team in a Patient Centered Medical Home Model.
  • Effectively verifies patient medication orders for appropriateness and accuracy. Provides medication reconciliation and evaluates medication appropriateness and completeness with respect to patient data, including age, height, weight, diagnosis, and other pertinent patient information.
  • Utilizes clinical pharmacy skill, including contraindication and duplicate therapy checks, in assessing and formulating recommendations to providers ensuring quality and economical drug therapy is performed daily.
  • Reviews chart to facilitate pro-active problem solving, productivity and team work.
  • Provides concise, applicable, comprehensive and timely responses about medication queries from providers and patients.
  • Actively participates in development and implementation of related performance/quality improvement and patient safety activities. Reports medication errors and adverse reactions promptly. Participates in clinical guideline development, disease management and drug use programs.
  • Follows HealthLincs procedure on the referral process and documentation of care provided.
  • In collaboration with providers, may order laboratory tests necessary for monitoring outcomes of medication therapy.
  • Provides information, education, and counseling to patients about medication-related care.
  • Participates in multidisciplinary reviews of patients progress.
    • Communicates with payers to resolve issues that may impede access to medication therapies.
    • Provides individualized health promotion and disease prevention.
    • Maintains compliance standards in accordance with the Compliance policies and the Code-of-Conduct. Reports compliance problems appropriately.

Qualifications

REQUIRED QUALIFICATIONS:

Education/Training

  • Doctor of Pharmacy (Pharm.D.) degree

Experience

  • At least 2 years in pharmaceutical services or completion of a PGY1 Residency Program
