JF&CS is now seeking a Full-Time Program Coordinator, Memory Support Services.


PURPOSE: 

The Memory Support Services Program Coordinator is responsible for the organization, delivery, and tracking of programs that support older adults living with memory loss or dementia and their family caregivers. These services include: 

  • Brain Health Bootcamp - Weekly small-group classes for older adults in the early to moderate stages of cognitive decline, focused on brain and body exercises, memory strategies, and social connection.
  • Music & Memory-A personalized music program that uses MP3 players or Simpl devices to reduce agitation, anxiety, and isolation for individuals with moderate to late-stage dementia.
  • Therapeutic Pets - Robotic pets provided to homebound clients with dementia and to children and adults receiving JF&CS clinical services, used to reduce stress and enhance well-being.
  • Memory Lane TV-A streaming platform offering therapeutic video programming to improve engagement, quality of life, and behavior for individuals with cognitive decline.
  • One Good Deed Dementia Respite Program - A volunteer-based respite program that matches trained community members with individuals living with dementia to provide social engagement and relief to caregivers.

This role ensures these programs are implemented with excellence and compassion, while maintaining strong relationships with caregivers, contractors, volunteers, and internal staff. 


ESSENTIAL DUTIES AND RESPONSIBILITIES include the following (other duties may be assigned): 


Program Coordination & Implementation 

  • Oversee the day-to-day operations for all Memory Support Services programs listed above.
  • Assist supervisor with coordinating Brain Health Bootcamp, including participant intake, Zoom/transportation assistance, attendance tracking, contractor support, room setup, and payment processing.
  • Oversee operations of Music & Memory and Memory Lane TV, including:
  1. Working with caregivers and volunteers to select personalized music or video content.
  2. Downloading content to MP3 players, Simpl devices, or streaming platforms.
  3. Providing clear written and verbal instructions for device use.
  • Mailing or delivering devices and offering additional support via Zoom, FaceTime, or in person.
  • Distribute and manage robotic Therapeutic Pets for homebound individuals with dementia and clinical clients who would benefit from emotional support.

One Good Deed Dementia Respite Program 

  • Collaborate with One Good Deed staff to coordinate volunteer-client matches that offer meaningful companionship to individuals living with dementia and much-needed respite for their caregivers.
  • Plan and facilitate volunteer training sessions and planning meetings in partnership with the Contract Memory Support Services Specialist and other Aviv team members.
