Salary Range: $61.76 - $75.68 Hourly

Location: Based at HealthPoint Administration but ability to float to all HealthPoint locations is required, potential for flexible schedule

APP POSITION FOR NEW INTEGRATED CARE PROGRAM FOR COMPLEX AND OLDER ADULTS
HealthPoint is hiring a 0.50 FTE position for an advanced practice provider to work with an interprofessional team to care for complex and older adults. This is an innovative, new role for an APP who has an interest in complex care management, and collaboration between social workers, primary care providers, and other care team members. The APP provider will be expected to travel between 10 medical clinics throughout the Puget Sound region in addition to occasional home visits, with travel reimbursement available. An ideal candidate would also have an interest in quality improvement, population health, and an understanding of chronic disease risk closure.

Position Summary:

Responsible for provision of comprehensive primary care services in a primary medical care, ambulatory clinic setting, and in Health care for the Homeless facilities. Works independently and interdependently with other clinic providers.

Compensation is dependent on skills and experience.

Your contribution to the team includes:

• Provision of comprehensive primary care services for all ages in a primary medical care ambulatory clinic setting, and in Health care for the Homeless facilities working independently and interdependently with other HealthPoint providers.
• Provide care consistent with evidence-based medicine.
• Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals for service.
• Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verify and document that the patient understands diagnosis, treatment and follow up recommendations.
• Complete accurate and timely documentation in patient records and superbills.
• Participate in patient triage system.
• Perform minor outpatient procedures in accordance with the scope of practice and HealthPoint privileging policy.
• Provide consultation to other clinic providers either in person or by telephone.
• Consult with clinic physicians, specialists, or other health care professionals as appropriate.
• Administer medications in clinic as needed.

Must have’s you’ll need to be successful:  

• Graduate from an accredited nurse practitioner program and have a minimum of two years of experience with adolescents.
• Washington State nurse practitioner license; must be certified

Proof of vaccination for COVID-19 is required, prior to start.  HealthPoint does not accept the Johnson & Johnson COVID-19 vaccine as proof of vaccination.  If you have received the Johnson & Johnson vaccine, we ask that you provide documentation demonstrating proof of an alternate COVID vaccine or vaccine series.
