Job Duties
New River Health District is looking for a self-motivated, outgoing, and passionate professional to lead its population and community health program. As Population Health Community Coordinator, this position is charged with engaging community partners in current public health issues to develop an ecosystem of support to identify and address needs and opportunities.
The position will represent the health district to local agencies and governments while serving as an internal liaison, ensuring health department staff are apprised of projects, programs, services, and resources throughout the district service area. The ideal candidate will have a solid understanding of the unique needs of the citizens and communities of the New River Valley, a passion for public health and related services, a proactive and holistic approach to addressing identified needs, and an ability to engage and foster relationships with individuals and agencies in improving the lives of NRV residents.
The Population Health Community Coordinator serves as the Healthy Communities Coordinator for the local health district’s efforts to work on developing and implementing environmental strategies to improve population health. Represents the health district at community events and assists, as necessary, in the coordination of health community activities. Supports and reinforces healthy behaviors in the district by working with community coalitions, schools, businesses, city leadership, urban designers and other local organizations and partners to build support for lifestyle change.
Routine duties and responsibilities will include: having primary responsibility for the district social media presence; representing the district on various community workgroups, boards, and/or committees; advocating for district needs and the needs of the communities we serve; provide health education information to diverse groups and individuals including professionals and community members; administer grants and participate in the research for and writing of related proposals and reports; help coordinate human services efforts between partners; collect, analyze, and report on data; serve as a member of the district leadership team including participation in meetings, development of strategic plans, etc.; provide direct client services and referrals to include working with incarcerated individuals, individuals at risk or current using substances, and other marginalized populations; maintain district inventory of incentives and promotional items; seeking out health fairs and opportunities for the district to collaborate with partners and citizens; project management; et al.

Minimum Qualifications
• Knowledge of Health Education & Promotion: Understanding of population health data collection, community demographics, social/cultural environments, and chronic disease prevention. Ability to implement and evaluate health improvement programs and policies for diverse populations.
• Communication Skills: Excellent oral and written communication skills to effectively guide staff, educate internal/external stakeholders, and respond to data requests. Ability to develop and deliver training materials, seminars, and health education sessions
• Resource Management: Knowledge of procurement processes, budget monitoring, and ensuring proper use of resources. Ability to prepare grant applications, manage grant activities, and maintain compliance with funding guidelines.
