About the role
Overview
CFHI is seeking a compassionate and dedicated Homevisitor RN to provide education services to women and their families in Ocean County through the Nurse-Family Partnership (NFP) program. This role focuses on empowering families to achieve their health and life goals while fostering positive relationships within the community and organization.

Responsibilities
- Deliver nursing services aligned with the NFP model and Visit-to-Visit guidelines, carrying a caseload of up to 25 clients.
- Conduct home visits to assess and address the health and developmental needs of women and their families.
- Provide counseling and instruction to help families achieve targeted health and life goals.
- Build and maintain strong community relationships to facilitate client referrals and resource connections.
- Attend weekly team meetings, case conferences, and clinical supervision sessions.
- Mentor and precept new hires and support student nurses during home visits as needed.
- Ensure compliance with agency infection control and safety policies.
- Maintain professionalism in adherence to organizational policies on absenteeism and appearance.
Qualifications
- Education: Bachelor’s degree in Nursing required.
- Experience: 2-3 years of relevant nursing experience; home visiting or maternal/child health experience preferred.
- Skills: Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities.
- Language: Bilingual (Spanish-speaking) candidates are a plus.
